Alliance Physical Therapy Partners, LLC, together with its affiliated physical therapy practices (collectively, “Alliance”), strongly believes in maintaining the privacy of nonpublic personal information we gather from individuals who visit our websites.

Personal Information vs. Protected Health Information

When you use a website(s) owned by Alliance, we may ask you to provide us with certain personally identifiable information, including, but not limited to, your name, photo, address, date of birth, zip code, phone number and e-mail address (“Personal Information”), and we may collect information about your use of our sites or your other online activities.  We may also obtain Personal Information when you engage in e-mail, text or other communications with us.  Alliance does not sell its customer lists and will not sell your name to a catalog company.  It is Alliance’s general policy to require all third parties that are to receive any Personal Information to sign strict confidentiality agreements.

In keeping with the general informational purpose of these sites and the related communications, we collect Personal Information from you as an online consumer and not as a patient of affiliated physical practices.  This Privacy Policy describes how Alliance handles Personal Information.  By contrast, any information that affiliated practices may collect when they provide physical therapy services (“Protected Health Information”) is governed by a separate Notice of Privacy Practices that will be provided to any patient in the course of delivering physical therapy services.

Alliance Services 

Alliance collects and uses Personal Information to deliver superior service to you.  If you are a physical therapist, such service includes suggestions on improving your patients’ experience, growing your practice’s EBITDA, industry insights, as well as information regarding Alliance and its services.

If you are a past, existing or future patient of the affiliated practices, such service includes providing general health and wellness information regarding physical therapy.  As you know, physical therapy addresses issues, such as illnesses, or injuries that limit a person’s abilities to move and perform functional activities in their daily lives.

Information contained on Alliance’s website(s) is available to the general public and not for the purpose of providing medical advice or physical therapy services.  None of Alliance’s website(s) are intended to serve as vehicles for providing medical advice or physical therapy services.  Please consult your doctor regarding your specific diagnosis, treatment, and health questions.  If you are experiencing an urgent medical condition, call 9-1-1.

Anonymous Information and Tracking Technologies

When you use our websites, we may automatically collect certain information, such as where you browse within Alliance’s website(s), traffic and location data, and information about the computer and Internet connection.  This information helps us administer the website(s) and provide a better user experience, and we may connect this information to Personal Information that you provide to us or we collect from other sources.

While we use industry-standard technologies and measures to protect any data you provide to us, nothing can completely guarantee the security of transmission of data or information over the Internet.  For this reason, we cannot guarantee against the loss, misuse, or alteration of data or Personal Information that you provide to us, and we are not responsible and disclaim any liability for any damages or injuries which arise from such loss, misuse, or alteration.  You provide the Personal Information to us at your own risk.

Child Privacy/Notice to Parents

We recognize that protecting children’s identities and privacy online is important.  Alliance’s website(s) are intended for adults age 18 and older and are not intended to attract children.  Alliance will not knowingly collect Personal Information from any person we actually know to be under 18 years old.  If you are under the age of 18, do not use the site(s) or provide any information through the site(s).  We also encourage parents to talk to their children about safe and responsible use ways of using the Internet.

Opting Out
